An Ohio woman has been indicted by a grand jury after officials say she ‘severely’ neglected her children, leading to the death of her 2-year-old son.

McClatchy News is not naming the woman or the victim in order to protect the identity of the children involved.

On Aug. 6, police and the Cuyahoga Metropolitan Housing Authority responded to a Cleveland home to perform a wellness check, according to a news release from the Cuyahoga County Prosecutor’s Office.

When first responders arrived, they found the home in “deplorable” conditions, and a 2-year-old boy was found unresponsive, the Aug. 22 release said. The boy was pronounced dead at the scene.

Two other children, 9 and 11 years old, were found severely malnourished, officials said.

“It is unimaginable to think a mother would do nothing while her children starved,” Prosecutor Michael C. O’Malley said in a statement. “The number one duty of a parent is to provide care for their children and (the woman) failed in that duty. We will do everything in our power to get justice for (the boy).”

The two children were taken to a hospital to be treated.

Officials said the 2-year-old boy had a medical condition that was left untreated. His official cause of death has not been released.

The 33-year-old woman was charged with one count of involuntary manslaughter, one count of felonious assault and three counts of endangering children, documents show.

The woman was given a $100,000 bond.
